What types of health insurance do you accept?

We are predominantly a Private-Pay, Fee for service, Cash-based practice. At this time we also accept Medicare. We provide all our patients with a Superbill, which in many instances, allows you to receive reimbursement for Out-of-Network services provided.

Do I need a doctor’s referral?

At Functional Therapy and Wellness you do not need a Doctors referral to be evaluated and treated. Under Florida Law, you have the right to see a Physical Therapist through Direct Access. If you have a Doctors referral already you are good to go! 

What is your new patient procedure?

Our New patient procedure is as streamlined as possible. We require that you fill out a few forms before you are seen by the Physical Therapist. You can click the link below to obtain the forms.

Cash-Based Model Overview

Utilizing a cash-based, private pay  physical therapy model we work for YOU, not the insurance company. A cash-based model allows the therapist to engage in an agreement with you to provide physical therapy services in a manor that both parties decide upon that will allow you to reach your rehab and treatment goals effectively.

Full transparency: No surprise bills from your insurance company 2-3 months.

Cash-based ensures you are not limited to what the insurance company says THEY WILL PAY FOR, but what YOU NEED! In some instances, you can personally file for reimbursement from your insurance companies which will reduce your out-of-pocket expenses. Most insurance companies will provide reimbursement for services received "out of network", except for, Medicare, Medicaid, and some HMO's. We will assist you with this process. 

Why Cash-Based?

We want to provide the care you need and don't want to be limited by third-party payers and neither do you! Often times third-party payers limit a patients progress and recovery and hinders our ability to treat you due to confining requirements and interfering with treatment.  Benefits for the patient in Private Pay:

  • You Spend the entire treatment session with your doctor (not sharing time with 3-4 other patients)

  • Efficient and timely treatment (you don't need 3 visits/week for months on end or miss work to attend PT)

  • Private Treatment Room (discuss your problems in secure, private environment)

  • We address Injury Prevention and performance goals (Most insurance companies only cover PT that is "Medically Necessary" so you cant use your in-network benefits for preventative measures, pain management, or return to sport activities)

  • Education and instruction for future success. We want to educate, empower, and improve your quality of life when you are in pain but we also want to give you the tools to prevent future injury and disease. We will give you the tools to prevent future pain and dysfunction, instead of just treating pain after it occurs. 

Although we do not take in-network insurance, we do offer Superbills after every session for you to obtain out-of-network reimbursement and submit payment towards your deductible. Actual rate of reimbursement depends on your particular plan. In addition, we accept all FSA and HSA cards so that you can utilize the benefits given to you from your health coverage while still receiving superior care.
